The Healthy Aging Blueprint™ is a comprehensive in-home nursing assessment designed to look at the whole person—not just a diagnosis or medication list.

During this visit, I take the time to understand your health history, daily routines, current concerns, home environment, and goals for the future. Together, we identify opportunities to improve your health, reduce risks, and make practical changes that can help you continue living safely and confidently at home.


What Is the Healthy Aging Blueprint™?

The Healthy Aging Blueprint™ is approximately a two-hour, in-home visit with a Registered Nurse.

Unlike a typical medical appointment, there is time to slow down, ask questions, look at the bigger picture, and identify concerns that may otherwise be overlooked.

We’ll talk about what is working well, what has become more difficult, and what could help you feel stronger, safer, and more supported.

Your assessment may include:

  • Comprehensive health and medical history review
  • Medication and supplement review
  • Vital signs and weight
  • Nutrition and hydration assessment
  • Mobility, strength, balance, and fall-risk evaluation
  • Home safety review
  • Review of chronic health conditions
  • Sleep, energy, and daily routine
  • Preventive health considerations
  • Healthcare providers and current care plan
  • Recent hospitalizations or changes in health
  • Current concerns or symptoms
  • Family or caregiver concerns
  • Personal goals for maintaining independence and quality of life

You’ll Leave With a Personalized Plan

The assessment is only the beginning.

Following your visit, I’ll organize what we’ve discussed and provide you with your Personalized Healthy Aging Plan™.

Your plan will outline clear, practical recommendations based on your individual needs.

These may include simple shifts related to:

  • Medication organization and safety
  • Nutrition and hydration
  • Strength, mobility, and fall prevention
  • Home safety
  • Medical follow-up
  • Questions to discuss with your physician
  • Preventive care
  • Community resources
  • Additional support that may make life at home easier
  • Strategies to help you maintain independence

The goal is not to overwhelm you with a long list of changes.

The goal is to identify the most important next steps and create a clear path forward.
